Please make sure we have an up to date Individual Health Care Plan if applicable. 

If your child needs medication during the school day, please complete a Medical Permission Form.  Please note we can only administer medicines that are prescribed by a doctor and need to be taken 4 times a day.  Medicines must be brought to and collected from the office by an adult.  All medicines must be in their original packaging.
